Not sure of the size, but the manufacturer is Harry Davis.  This
fascinating little tidbit is from
http://www.wiard.com/Packaging/standard.htm, as well as other cool
analog factoids.  Coolest idea from Wiard: Using the CA3096 for an
exponential convertor, with the PNP pair used as an expo convertor for a
tri/square oscillator (13600, tomg?), and the other NPN transistors used
as a heater circuit (as per AN-299, Moog Prodigy/Source, Serge NTO).
