Preamp Issues

I've built myself a preamp out of electronotes.  The problem I'm having
with it is that with no input, it oscillates wildly.  Not only that, but
with an audio input in the second stage, at regular instrument level, the
signal has oscillations mixed in.  What can I do to fix this?  Less gain?
Caps in the audio path?
