I have been using 6N138 with no problems.
I have got them from Asian suppliers and also Digikey in USA, branded QTC.
texas Instruments no longer make them I believe - not enough margin now.

would like to hear what others are using. Some optocoulers do not have
enough current gain, it isn't always just speed i think......
