It gets even easier if you can find something like a 3-5 power
microscope with crosshairs. You can then make a setup like this:

Where you peer through the crosshairs microscope from the top, and
work the drill lever to drill from the bottom, sliding the PCB around
so you line up the crosshairs in the center of pads, having previously
lined up the drill to hit the crosshairs exactly. Humans are -*VERY*-
good at lining up crosshairs in circles.
This is fast and efficient. It's just very hard to find a low power
microscope with a crosshair reticle.
